Care.data is a NHS England national programme.

Care.data will collect both identifiable and non-identifiable information from GP Practices, such as symptoms, diagnoses, drugs and lab results. Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected.

The information collected is to be managed and anonymised by the Health and Social Care Information Centre (HSCIC) and will be utilised for planning and research purposes.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please speak to practice staff.

To find out more about the Care.data scheme, you can visit the Care.Data NHS England website.

Please note that the practice does not have any involvement in this scheme including design, public consultation, data storage or use of data. Therefore, the practice supports the decisions of all patients, whether opting in or opting out of the programme.
